I have recently visited the area.

The trains on that route are not the super fast TGV trains but are comfortable. Reservation is required for all journeys - the trains are often full. I advise that you plan your journey and make reservations as far in advance as you can - reservations open 90 days before travel.

There are no direct trains - a change of train is required en route in Toulouse.

Be aware that it is a very long journey, between 8 and 11 hours, so you may prefer to take an overnight train.

As an alternative to that very long train journey, you may prefer to fly from Nice to Toulouse with Air France then take the train for the remaining 2 hours journey to Lourdes.

I hope that helps you.
