What are the transportation choices from Santiago to Lourdes.
Flights : They all seem to route me first to Madrid or Paris
Are there any trains or buses

You can do the journey by train travelling through Northern Spain without going anywhere near Madrid or Paris, but is is a bit complicated. See my post in the %26#39;Train to Lourdes%26#39; topic concerning the timetable search. The train journey would last between 18 and 23 hours.
I don%26#39;t know if there are any bus lines operating across that part of Spain towards Biarritz/Bayonne from where the local trains run to Lourdes.
